  • When is the right time to start treatment?
  • What type of treatment is recommended?
  • How long will the treatment take?
  • What will the treatment cost?

Diagnostic records will be obtained, and Dr. Polczynski will complete an exam to gather key information.

What Are Diagnostic Records?

Diagnostic records help us assess your dental needs:

  • Photos of the teeth and face
  • Panoramic x-ray
  • Cephalometric x-ray 

Our team uses this information as the first step to create the best treatment plan for your needs.

Treatment Plan

After reviewing all diagnostic records and any concerns you may have, Dr. Polczynski will sit down with you to discuss your treatment plan.

She will review all of the diagnostic records in detail and explain all treatment plan options, including any pros and cons. It is our primary goal to make sure that the patient’s feel comfortable and understand their plan in detail.

Cost of Treatment

Treatment costs vary depending on the severity of the issue. During your initial visit, our treatment coordinator will discuss fees and available payment options if treatment is required.

We offer flexible payment plans to accommodate your budget and accept most insurance plans. We strive to make orthodontics affordable, so you can achieve the smile you’ve always wanted.

What Does My Investment Include?

Your treatment investment covers:

  • Orthodontic records: x-rays, digital scans, and photographs to establish your treatment plan.
  • Active treatment: whether you choose early treatment, braces, or clear aligners, all procedures, appliances, and office visits are included. Appliances are durable if properly cared for, but if they are damaged or lost due to misuse, we may charge a replacement fee.
  • Final retainers: Retainers will be provided at the end of treatment. There is a replacement fee for any lost or damaged retainers.

What Doesn’t My Investment Include?

Your investment does not cover:

  • Services performed at other dental offices.
  • Accidental injuries or issues, such as TMJ complications or the need for additional surgeries, which may result in extra charges to complete treatment.

Assignment of Benefits

We accept most insurance plans and can help reduce your out-of-pocket expenses by accepting the assignment of benefits. If your plan doesn’t allow the assignment of benefits, we’ll gladly file the claims directly for reimbursement.

If there are any changes to your insurance policy during treatment, please notify us immediately so we can adjust the contract as needed.

To help us maintain our schedule, please arrive on time for your appointment. Late arrivals may cause delays as we try to stay on track. We appreciate your understanding and cooperation, as this allows us to serve all our patients effectively.
